This was a joint ADASS, NHS East of England and Department of Health East of England conference.
The conference included a range of speakers from across the Region and a variety of workshops to demonstrate the continuum of support for people living with a long term condition.
In addition to the conference, there was an exhibition of telecare and telehealth technology providers, including examples of the services and support already being provided in the Region.
The conference was targeted at professional staff and managers who are involved with supporting people who live with a long term condition to be in control of their lives and to be better supported to manage their own condition.
It was also of interest to individual people who are living with a long term condition and to carers who are involved with supporting a person with a long term condition.
