12 November 2009 - Long Term Conditions Development Day - 'all you ever needed to know about personal health planning....'

This was an NHS East of England Event co-hosted by EDC

Objectives of the day were:

  • To engage and collaborate with all stakeholders (service users, carers, 3rd sector, commissioners, providers across health and social care)
  • To gain the views of stakeholders and offer a chance to network
  • To facilitate the development of a programme to support the implementation of personal health planning across East of England

The day began with 'an introduction to what personal health plans mean to me - a service user perspective' with Mitzi Orr and her GP, Dr Vincent Forte. The day also featured an interactive market place and a series of mixed discussions group - the subject 'How are we going to work together to implement personal health planning and supported self care for the 1.6m people in the East of England with Long Term Conditions?'
